Congestive Heart Failure Program


New Program Assists CHF Patients


Glendale Memorial Heart Center is pleased to introduce our new Congestive Heart Failure (CHF) Program. Designed to assist patients living with this disease, the CHF program is provided as a complimentary service and is funded through generous grants.

This free program is overseen by a dedicated nurse practitioner, who is available to personally meet with you and further explain the program and answer any questions you may have.

Heart failure is the most common diagnosis for patients over the age of 65 and is often a progressive illness that can greatly limit quality of life and ability to manage day-to-day activities.

Highlights of the CHF Program:

  • Once discharged, weekly follow-up calls to patients for the first month
  • Follow-up calls every other week during the second month, and then on an as-needed basis
  • Coordination of care with the patient’s cardiologist or primary care physician
  • On-going education regarding daily weights, medication compliance, early warning signs/symptoms, diet and exercise
  • Individual attention by the Heart Center’s nurse practitioner to address any needs identified during follow-up calls
